Make a Referral

1. Phone our Client Engagement Team and they can take your details over the phone.
2. Your GP, Support Coordinator, Nurse Navigator or other healthcare professional or support person can complete the referral form on your behalf.
3. You can self-refer for services at any time and do not require a referral form if you are a NDIS participant or self-funding your appointments. Please note that we usually have a waitlist for our services and it can be difficult to provide estimates on wait times. We encourage you to leave your details with us so we can get in contact with you if we have a cancellation spot available.
